Area Chairman Forms

Now that Vivace, the online Festivals Management System, is required by NFMC for the operation of all Festivals, some of the forms below will no longer be required.  Vivace will take care of all record keeping and reporting of results to the state Festivals chair.  Other forms intended for judges, child protection policy and liability insurance are still valid.

This page is intended to provide information and the necessary forms for your Federation Festivals Area. These forms are the standard Federation Festivals Packet sent to you, the Area Chairman, and will be updated when National updates its website.  Always check forms for the current Federation Festivals year in the heading or footnote.  Forms should be dated 2022 or 2022-2023.

NOTE:  Newly added forms include the current Child Protection Policy and the Application for Liability Insurance.

This page is not intended for the individual Junior Counselor.  A separate page has been provided for the individual teacher.

You will notice that the Report of All Performers, the Summary Report and the Teachers List have not been included. 

These will be sent to you directly from the State Festivals Materials chairman.
